That's a machete wound. There's a no way a kite line can cut that deep unless it was super taut and strong enough to withstand a motorcycle rider going fast into it.

Depending on the speed of the motorcycle, the damage can be much greater, and the rope as well. I would guess that in this photo the rope had difficulty making a deep cut, as it must have gone slowly or something like that.

Adding to this, there's a thing called 'injuries incompatible with life' that is used by medical professionals where they essentially decide whether it's even viable to help the person, or if their life would be worth saving (since the aftermath of their recovery could be endless misery) and in those cases they don't attempt to save the person. I don't know if this is true of every country, hospital, and doctor, but it is a common practice and it's likely if he's there they believe he's able to be saved and continue on. I've been consistently amazed with what medical professionals are capable of in terms of human reconstruction.
