He was conscious the whole time, that higher pitched noise you can hear towards the end when the guy filming is yelling "stop this fucking truck" is him shrieking (not really the right word but I'm not sure how else to describe it). The truck tyres are pinning him by the legs and lower torso, he's facing down and upper torso, right arm and head are "free" of the wheels (unfortunately for him as it prolongs the ordeal).
He was one of the protestors or rioters and had originally crawled under there when it was stationary, unsure if he was trying to interfere with the truck trailer connections to detach it or trying to get into it, but then got caught up as it rolled forward. There is another video somewhere showing more of the earlier stages. IIRC there was a board or box between part of him and the road.
