On May 7 , a surveillance video was exposed online: when a crane was closing in after work, a large hook fell from the top and hit the control room. The crane driver was unfortunately killed!

Recently, the Shenzhen Emergency Management Bureau released the cause of the accident:

At 18:10 on May 7, at a construction site in Shenzhen, a driver was operating a crane truck in the control room. Another worker was standing outside the control room chatting with the driver . During this period, the driver was operating the main hook and hook of the truck crane. The auxiliary hook was lifting and lowering without load. Suddenly, the auxiliary hook fell, hit the top of the control room and then hit the head of the person involved, causing the driver to die on the spot.

Preliminary analysis of the cause of the accident: The auxiliary hook height limiter was not installed on the top of the jib of the truck crane involved . The driver involved operated the truck crane without the guidance of personnel. During the operation, the auxiliary hook rose rapidly and overturned. The auxiliary hook crossed the top pulley of the jib and the pulley anti-separation groove device. The falling auxiliary hook penetrated the top of the control room and hit the head of the driver involved, causing the accident. (The direct cause analysis conclusion shall be subject to the accident investigation report)

In Loilo City, Philippines, 18-year-old Jomar Junco was found dead after he was found hanging out of a chorizo food processor

Police Major said that Junco initially got his right hand stuck inside the machine and was not able to pull it out and Junco was then pulled in by the grinder. "He was not grinded. He was just stuck". Investigators were told that Junco didn't not know how to operate the machine. When one of his co-workers stepped out of the room, Junco tried to operate the machine by himself and was killed quickly after.

On November 2020-, a 65-year-old worker died at the KAMAZ PJSC factory in Naberezhnye Chelny, Tatarstan, Russia after getting caught in a spinning machine. He was trying to sharpen a machine part but was sucked in after his jacket gets caught on the wheel, resulting in being shredded

This most commonly happens by your hair or clothes getting caught in it when you're too close to the machine. Make sure to workplace safety guidelines and wear appropriately fitted clothing and keep your hair put up and out of the way to prevent injuries like this

Sliding bulldozer kills two shipyard workers (at 0:29)

A work accident at the Tanjunguncang shipyard, Batam City, Riau Islands Province (Kepri), killed two workers because they were crushed by a bulldozer, Wednesday 8 March 2023. The work accident was recorded in a video that went viral on social media.

In the video, two workers can be seen talking near a broken down pick-up truck in the Tanjunguncang shipyard area, Batam. Meanwhile, beside him, a yellow bulldozer was seen passing along the incline.

At one point, the bulldozer seemed unable to move forward and instead slid down or reversed uncontrollably until it ran over the two workers at the Tanjunguncang shipyard, Batam,. One victim did not move after being run over by a bulldozer.

Not only did it run over two workers, the bulldozer also hit a black pick-up truck that was breaking down. The front body of the car appeared to be damaged due to the collision.

The two workers who were victims of being run over by the bulldozer were identified as Anni Aslimin (41) and Richard Sirait (56).

Batuaji Police Chief, Commissioner Restia Octane Guchy, confirmed that the accident occurred at the Tanjunguncang shipyard company.

"(The bodies of) the two victims have been taken to the Bhayangkara Regional Police Hospital in the Riau Islands," he told the media.

The Tanjunguncang shipyard is still one of the areas in Batam that is prone to work accidents. In fact, on the same day, a work accident also occurred which also killed two workers.

I didn't see a lot of information on this online, so I figured I'd share the story here. None of this writing is my own.

Industrial diver Alex Reed Paxton plunged into the bowels of Columbus' Lake Oliver Dam on Oct. 27, 2020, and never got out alive.

As he descended into the water along a floodgate chain looking for damage, his left arm was sucked into a high-pressure pipe, its valve inadvertently left open, and the suction's crushing power suffocated him.

Pinned by 850 pounds of pressure, the 31-year-old lost consciousness, and died in minutes. His body did not come free until workers found the valve control and released it, about 30 minutes later.

The 10-inch opening to the pipe that snagged Paxton's arm pulled water from a vertical space within the dam, called the headworks, where Lake Oliver's backwater flows through when the chain pulls open the dam's metal headgate.

The pipe, called a priming pipe, feeds water to what's called the penstock, another pipe about 20 feet in diameter, which at high velocity sends water to the turbines to generate electricity.

Opening the headgate to power those turbines first requires “priming” or “watering up” the penstock, Butler said. That requires opening valves to fill the penstock with lakewater.

Before a diver descends into the headworks where Alex Paxton was working, all valves controlling the flow to the penstock have to be closed, otherwise the differential pressure can trap the diver.

Georgia Power through its lockout-tagout safety checklist was supposed to have closed two valves where Paxton was inspecting the gate chain.

One penstock valve was a flapper, like the kind used in a bathroom toilet, and the other one was a gate valve controlled by a wheel, like on a water spigot.

First filed in Muscogee State Court in March 2022, before being moved to the federal level, the lawsuit accused Georgia Power not only of failing to close both valves, but of neglecting to note the gate valve even existed, on safety forms used at the time.

The utility showed the dive team from Glenn Industrial Group of Charlotte, N.C., a lockout-tagout list that didn't have that valve on it.

The flapper valve was noted on the form, and it was closed during the tag-out safety protocol.

The wheel controlling the other valve was behind a locked gate only Georgia Power staff had access to, the lawsuit said. That's what took so long to free Paxton.

Georgia Power since 2018 had contracted with Glenn Industrial for maintenance work on the dam.

In depositions taken for the lawsuit, witnesses said the company had a safety meeting with Glenn Industrial on May 27, 2020, when the gate valve was shown to the dive team.

Records showed Georgia Power broke the headgate gate chain to its Unit 2 turbine on Oct. 21, 2020, a week before Paxton's death.

With no mention on the tag out form, the gate valve noted during the May safety meeting still was overlooked five months later, when Paxton made his dive.

Georgia Power later corrected that oversight.
