SL-1 was a nuclear reactor near Idaho Falls that caused the death of three men, Richard Legg, John Byrnes, and Richard McKinley, the three men were technicians sent to SL-1 for basic maintenance but while performing the maintenance which is attaching the control rods to the control rod drive, but the Control rod drive lifted to a height of 20 inches in 0.5 seconds caused the reactor to go into a state called Super-Critical. The core's power level went to 20,000 megawatts in just 4 milliseconds.

The heat from the reactor instantly vaporized the water causing the launch of the drive pinning Richard Legg to the ceiling, John Byrnes was pinned under debris.
