The guy in surgery is a suicide survivor who put a gun under his chin and flinched (tilting his head back) as he pulled the trigger. There was a thread about it a long time ago on the ogrish forums. The surgeons did an amazing job, but he had a meandering scar that ran down the middle of his face.
