He is contorted and compressed in such a way its restricting his diaphragm. Leading to a rare form of suffocation called compressive asphyxia. I'm of the opinion that this asphyxia caused a Tonic-Clonic seizure and maybe even some vomit mixed in with the foam coming from their mouth and nose.
