I'm from New York, and the location and danger of the third rail is quite common knowledge that you learn at a young age so you don't end up like the deceased. From her positioning it seems she took a headfirst tumble and just won the shit lottery. She might have been impaired, but I am speculating. The news story said it might be that she was attempting to retrieve something. Who knows...
