The problem is, people put it under their chin, pointed in the "correct" direction, but the gasses expelled right before the shot comes out pushes their head back and it just takes out their mouth and eyes. "You were a failure in life and you can't even correctly kill yourself, Jerry" --Newman

It's not actually the gas, there's not enough time for it to influence the position of the jaw and skull before the cloud of pellets blast through them; it's the natural human instinct to lean away from the end of "the pipe that's about to make a very loud noise and destroy my brain" combined with the readjusted aim of someone trying to reach the shotgun trigger.

A lot of people, especially shorter / smaller people like this Asian man, don't realise how long and unsuitable a shotgun is for shooting themselves until they actually try to angle it correctly. The old school method is to place it on the ground and action the trigger with a toe, that way the barrel's still aiming directly up towards the back of the skull.
