Your pivot point changes suddenly when stepping onto any previously moving surface, and he made the terrible judgement call of holding his kid, who probably weights 40 pounds, in his arms as he did so, and it fucked with his center of gravity enough to make him try to take a step down to the step below, couldn't recover, and they both fell.
