Most of them don't even know how serious it is until the end of the video. Even then, they're trying to figure out what exactly happen. And they're kids, they're probably scared or panicked or just straight up confused. Even at 30, if one of my buddy's came running up to me screaming about their bleeding ball sack, it'd take a moment or two for me to try to get a handle on the situation.

But ambulances are expensive. Kid isn't suffering any damages that puts his life at risk so he can just ask for a ride to the emergency room. I would do that even if I were to need the ER

The reason why they don't immediately jump up is because they're skaters and they probably see like one split head and one twisted leg per session and they usually walk it off. Is this a congregation of Einsteins? No. Are they necessarily shitty friends? I don't think so.
