The tortured guy has a clown tattoo, which in Brazilian mob culture signifies 'cop killer.' So the cops are 'removing' it. They chose a brutal method, sanding his skin with a knife. This extremely painful process makes the skin extra sensitive and removes a layer until it bleeds, as seen at the end of the video.
