Just a quick medical input: A Dehisced wound is one which has been previously closed following an injury or surgery, but which has split open again because the edges of the wound failed to heal correctly, or where there hasn't been enough skin to close the wound properly.
Sometimes this happens while still stitched, other times it can happen after they're removed. Normally, the patient would have modifications made to the wound, to reopen a little fresh skin on both sides, and then have it stitched again. Swabs would also be taken to determine if any infection is present on the site prior to doing this, so that it can be dealt with before closure, to prevent this happening again.
If there's insufficient skin to re-close the wound, a small graft would be taken from elsewhere on the body, and used to extend the skin, permitting the wound to be safely closed.

They can, but in the context of wound management, if it's possible to re-close a wound using the existing skin, then that would take priority. A graft would only be performed if it was unavoidable. Unnecessarily carrying out a second operation to take a skin graft from another area, would complicate matters, because it would leave the patient with a second open wound which would also need to heal.
