what are you confused about in the picture ? it looks like the shot just hit the lower anterior mandible , and went up the front of the eye, which missed the lethal parts, ,, outside of the X ray the face probably looks like a bloody pulp so there's a breathing tube inserted into his throat so he doesn't drown in his own blood

My guess is he shot himself near mouth area, probably under the chin. The tube looks to be added my doctors to keep him breathing and not suffercate to death. It caused major fracturing but wasnt strong enough to go up above eye socket and kept him alive. This is just an educated guess
