I've always wondered how could these people be still alive at this stage of their disease (I mean with their brain exposed to air and all possible bacteria).

I’ve always assumed that it’s not a question of “alive or dead” it’s more “what is still alive”. If an infection effects his temporal lobe, he will be hyper aggressive, completely different person, but could still be alive to witness it. My guess is ol chap here is alive but running on fumes. Probably isn’t even registering pain, if any.

There are brain eating disease in some water supplies. Disney owns a park that is now closed due to the water causing this. It kinda looks like a gleoblastoma, that tends to infiltrate the tissue of the brain, the matter just changes color. Possibly syphilis or herpes.

Most diseases don’t have a chance to kill you, your body does it first. My guess is his immune response/diseased areas isn’t strong enough or spread out enough to cause something like a brain failure or heart attack. He probably didn’t last the day
