youd be suprised how much a lot of self harmers get away with without being involuntary committed. they do a lot of first aid at home. if they are extreme, they most likely are frequent flyers at the hospital and there is only so much the hospitals can do at a certain point for people who dont want help, especially if they “promise” they arent going to do it again. theres a girl i follow on twitter who slashed both her legs open and her arm that was just sent home with staples even though she od’d and lost a lot of blood.
