So what ended up happening? Did he start aiming his gun down due to some malfunction? it seemed like he was hit but in the video I don't understand, is this sort of risk common when shooting this sort of gun, or holding it vertical as apposed to horizontal, etc...

You can see him lose control of the weapon once the ammo belt becomes so short that the guy to the right, who is helping with holding it, is no longer assisting with countering the force needed to handle the two-handed LMG.

He was weak. This is a common risk when using a two-handed weapon one-handed and not being built like Sylvester Stallone. The recoil caused his finger on the trigger to continually press down because that was the only grip he had on the weapon until he managed to put two hands on it after the fact and was able to release the tension on his right hand.
