After thinking about it for a little bit:
1. Probably had done this climb countless times prior, trying to set a personal best record – this routine might've fostered complacency, where he was so hyper focused on the climbing part, that he didn't even realize he hadn't harnessed himself up beforehand.
2. Similar to #1 except the only difference being, he knew he wasn't harnessed up, but when he got to the top, he let go as a force of habit (from doing it so many times in the past) and probably instantly realized "oh shit" as he was letting go.
Complacency and force-of-habit cultivated from a routine can sometimes lead to disaster.

Your number one is more likely. He never even. Realized he wasn't clipped in. Totally forgot. Your number too is almost impossible. There's no way he realized he wasn't clipped in part way up and just kept going. I've been a rock climber inside and outdoor for more than a decade. NOONE would realize this mistake part way up the wall and just keep going. I've seen it multiple times where something like this happens and usually the whole gym responds immediately to help the person down without injury.
