Climbing a fake wall inside a building but too proud and dumb to use a harness. Always wonder what these guys are thinking as they're falling.

Dude he's wearing a harness, his mistake is that he forgot to clip into the auto belay. You have to do it every time you climb the wall and if you've been climbing for years and done the wall a thousand times sometimes you just make a mistake and forget.
There is no auto belays at my gym for this reason. You need to have an actual human being be your partner and they belay you with you attached to them. It's much less likely that two people fail to realize that something is wrong than one. Auto-belays are deadly for this reason.
Usually they have some sort or tarp covering the starting holds and the auto belay is clipped to that tarp so that you can't grab the start holds without first clipping yourself into the auto-belay. Obviously that's not the case here and this poor guy forgot to clip in and didn't even notice the whole time he was climbing. He got to the top and just let go of the wall expecting the author belay to catch his weight and lower him safely. He didn't even realize his mistake until it was too late.
