That doesn't necessarily have to be the case. There are lots of videos here that show it differently. Even while kneeling. I suspect that if the rope goes up the back instead of the front, it closes the necessary veins and arteries more quickly and you immediately fall unconscious. Similar to a karate chop to the neck. Your body will still show some reactions, but you will no longer feel them.
