Looks like it could be loss of tail rotor effectiveness (LTE). Recoverable, however pilot tried to pull up too hard to counteract the nose dip and the main rotor tilted too hard and cut the tail right off.

This had somthing to do with the rotor and seeing that the tail roto shut off it caused more problems
