A young race marshal, Frederik Jansen van Vuuren was killed instantly when he was struck at the 1977 South African GP. The driver of the car, Tom Pryce also died (see image) as a result of being partially decapitated by the fire extinguisher being carried by the marshal.

Apparently, the impact with Pryce was so great I believe the fire extinguisher landed in the car park embedded in a windshield!
