I think drugs had a major part to play here. Screams and squirming are natural automatic responses to pain, even with high pain tolerance this is severe pain. Can't be simply repressed.

I've actually heard the contrary, beheadings aren't typically that painful relatively thanks to the neck being the least sensitive to pain.
Give it a try. Try pinching the skin on your neck compared to somewhere else on your body like your arm. Most of the struggling is a panic reaction from the airway being obstructed by blood or the blade. This guy didn't have that because they mostly went in from the back.

My guess is they drugged him but it also doesnt make sense because cartels usually drug their victims to keep them alive for longer. Maybe he reacted to it differently?
