Many years ago I prepared risk assessments for water parks and I can tell you that such a situation can occur when two obese people, against the rules, slide at the same time and get stuck at the exit of the slide. The stress caused by the situation creates a toxic gas that builds up in the pipe due to the lack of an outlet. At this point, all it takes is a small spark for tragedy to occur.
