Two Los Angeles police officers will be disciplined for fatally shooting a woman who was being held hostage by a knife-wielding man in Van Nuys back in June of 2018.

The L.A. Board of Police Commissioners Tuesday voted unanimously that two of the three officers involved in the shooting that resulted in the deaths of 32-year-old Guillermo Perez and his hostage, 49-year-old Elizabeth Tollison, acted out of compliance with the department's policies.

The board's vote was in agreement with LAPD Chief Michel Moore, who issued a report on the incident. It's unclear exactly how Moore will discipline the two officers.

On the afternoon of June 16, 2018, officers responded to a homeless outreach center in the 6400 block of Tyrone Avenue on a report of a man with a knife assaulting a woman. After a tense standoff, officers fatally shot Perez as he held a large serrated knife to the neck of Tollison. She was struck twice by gunfire and died two days later at a hospital.

The bodycam footage of the shooting was released the following month.

According to Moore, officers had ordered Perez to drop his knife, but he refused, ultimately holding it to Tollison's neck. An officer had fired a beanbag shotgun during the confrontation, but it failed to stop Perez and the officers were forced to fire their handguns as the suspect pressed the knife into his hostage's neck, Moore said. Eighteen rounds were fired.

Perez was a documented gang member who had previously been convicted of robbery, police said.

The officers involved in the shooting were previously identified by the LAPD as Eugene Damiano, Andrew Trock and Cristian Bonilla, but Moore's report redacted all the names and it was not clear which officers were found to have used their firearms out of policy. Moore stated that the officers were not in the proper positioning at the time of the shooting.

Moore's report found that all three of the officers did not formulate a clear plan or did not take proper cover, per the department's policies. Moore also characterized the shooting as a "rapidly unfolding" tactical situation in which the officers had to make immediate decisions. One officer quoted in Moore's report said that because the beanbag shotgun was not effective, lethal force was necessary, and Moore said he concurred with the decision.

Police on Thursday identified both the suspect killed and officer who fired in a fatal police shooting earlier this month in Atwater Village.

Jose Antonio de Santiago-Medina, 59, was fatally struck by a bullet from the gun of Officer Paul Bernd, who had fired at the same time as another officer used a less-lethal beanbag round, Los Angeles police said in a news release.

The shooting occurred after Medina allegedly threatened at least two other men with a box cutter in the area of Edenhurst Avenue and Appleton Street the afternoon of June 6.

The victims, who didn't know each other, both reported their encounters to authorities, who responded to the area around 12:10 p.m.

Officers spoke to the victims and, about 12:25 p.m., found Medina, who matched the suspect description, standing on the sidewalk outside a home.

Medina initially complied with orders, but after about 30 seconds he stopped following directions and pulled a box cutter from his waistband, police said.

While wielding the weapon, Medina allegedly ran toward officers. One officer fired a beanbag round, while Bernd fired a lethal round, according to LAPD.

Medina was struck by both the bullet and beanbag and fell to the ground. He died at the scene, officials said.

New body camera video shows Los Angeles police officers' deadly confrontation with an armed man in southeast Los Angeles.

The man, later identified as David Flores, 36, can be seen firing at responding officers as they pull up to the intersection where he is standing. Officers shooting back, striking him. He goes down and is later pronounced dead at the scene.

The incident happened April 22 in the area of Figueroa Street and Gardena Boulevard around 6:45 a.m.

Officers were called to the area on a report of a family disturbance and a man with a gun. As they pull up to the intersection, the man can be seen immediately firing once in their direction, putting a bullet in the police cruiser's passenger side door.

The officers returned fire and Flores fell to the ground, though he did not immediately drop the gun. The officers can be heard repeatedly yelling at him to drop the weapon and he eventually complies.

Paramedics are there in minutes, but it's too late - Flores is pronounced dead at the scene.

Flores was a Gardena resident with a criminal history, including past convictions for arson, narcotics and battery, police say. Witnesses told ABC7 he had a reputation as a drinker and drug user with an attitude.

Passengers who were in the train carriage where a knifeman attacked another man pleaded with him to stop, the BBC has been told.

A man in his 20s suffered life-threatening injuries when he was stabbed while travelling on a train in south-east London on Wednesday.

One passenger, who did not wish to be named, said: "People were shouting, 'don't kill him, don't kill him'."

A 19-year-old has been arrested on suspicion of attempted murder.

The victim is in a critical but stable condition in hospital, British Transport Police (BTP) said.

The passenger said he was on the Southeastern train, which was travelling between Shortlands and Beckenham Junction while on its way to London Victoria, when he saw two people taking part in what "looked like rough play initially".

"Very quickly it spilled away from the seats on to the floor of the carriageway.

"At that point I saw a knife. A few other passengers saw it too and ran away from that area and came towards us," he explained.

The witness said he and other passengers then tried to plead with the attacker to stop.

"People were shouting 'don't kill him, don't kill him'," he said.

The BTP was first called to reports of two men seen in an altercation while entering a train at Shortlands station just before 16:00 GMT on Wednesday.

In footage of the incident, passengers in the carriage were heard gasping and shouting at a man, wearing a hooded jacket and a facemask, to stop.

The force said it had recovered a knife as part of its investigation.

According to data provided by the head of the Investigative Committee of Russia, Alexander Bastrykin, 40 people died as a result of gunshot wounds and 45 as a result of fire started by terrorists.

March 23, 2024 Russian President Vladimir Putin stated that the attack was "bloody and barbaric" and announced that March 24 would be a day of national mourning. Putin also stated that all the attackers had been detained and that the perpetrators were heading towards the border with Ukraine, where a shelter was prepared for them.

The mayor of Moscow, Sergei Sobyanin, called the attack a "great tragedy" and canceled all events in the city that were to be held on the weekend after the attack. Mass events were also canceled in other regions of Russia on these days.

The press spokeswoman of the Ministry of Foreign Affairs of the Russian Federation , Marija Zakharova , called this event a "cruel crime".

Yulia Navalna , wife of Russian oppositionist Alexei Navalny , who died a month before these events in a Russian gulag, wrote on Twitter: "What a nightmare at the Crocus hall. LAPD shoots man walking towards them with a knife (multiple POVs)

A 38-year-old man killed in a confrontation with LAPD officers in South Los Angeles in April was struck at least six times by police bullets and had detectable amounts of methamphetamine in his blood, according to a report from the LA County Coroner's office.

Daniel Hernandez was shot in the top of the head, and in the shoulder, abdomen, thigh, and upper back. The deputy medical examiner listed the cause of death as multiple gunshot wounds.

Hernandez's 14-year-old daughter sued the LAPD earlier this year, alleging the shooting was unjustified and the officer who fired had, β€œreckless violent and homicidal propensities.”

It happened April 22, 2020 after officers from the LAPD's Newton Division were dispatched to the scene of a traffic collision at 32nd Street and San Pedro, and after they arrived, Hernandez emerged from a wrecked car and walked toward the officers.

According to an edited LAPD video presentation posted on YouTube a 9-1-1 caller told police dispatchers that the man who caused the crash appeared to be trying to stab himself, but it was unclear if that information reached McBride or her partner before they began interacting with Hernandez.

β€œDrop the knife!,” she yelled three times as Hernandez walked into the center of the intersection. More than a dozen bystanders can be seen on the sidewalk. β€œStay right there,” McBride commanded.

Hernandez paused, then stepped towards McBride's position with his arms outstretched to the sides at waist level. β€œDrop it!,” McBride yelled, then fired twice when Hernandez started to advance. The shooting was captured on both the officers' body-worn-video cameras and by at least one of the bystanders who was recording with a cellphone.

Hernandez collapsed to the ground, then began to get up. McBride fired four more times and Hernandez fell to the ground face down and stopped moving. None of the other officers at the scene fired their weapons. About a minute and twenty-five seconds elapsed between the start of the video and first shot fired.

Beirut Explosion (9 different angles) (Infos + Results)

On 4 August 2020, a large amount of ammonium nitrate stored at the Port of Beirut in the capital city of Lebanon exploded, causing at least 218 deaths, 7,000 injuries, and US$15 billion in property damage, as well as leaving an estimated 300,000 people homeless. A cargo of 2,750 tonnes of the substance (equivalent to around 1.1 kilotons of TNT) had been stored in a warehouse without proper safety measures for the previous six years after having been confiscated by Lebanese authorities from the abandoned ship MV Rhosus. The explosion was preceded by a fire in the same warehouse.

The blast was so powerful that it physically shook the whole country of Lebanon. It was felt in Turkey, Syria, Palestine, Jordan, and Israel, as well as parts of Europe, and was heard in Cyprus, more than 240 km (150 mi) away. It was detected by the United States Geological Survey as a seismic event of magnitude 3.3 and is considered one of the most powerful non-nuclear explosions on record.

Fire and first explosion

Around 17:45 local time (14:45 UTC) on 4 August 2020, a fire broke out in Warehouse 12 at the Port of Beirut. Warehouse 12, which was waterside and adjacent to the grain elevator, stored the ammonium nitrate that had been confiscated from Rhosus alongside a stash of fireworks. Around 17:55 local time (14:55 UTC), a team of nine firefighters and one paramedic, known as Platoon 5, was dispatched to fight the fire. On arrival the fire crew reported over the radio that there was "something wrong" as the fire was immense and produced "a crazy sound."

Damage

The damage from the blast affected over half of Beirut, with the likely cost above US$15 billion and insured losses at around US$3 billion. Approximately ninety percent of the city's hotels were damaged and three hospitals completely destroyed, while two more suffered damage. Dozens of injured people brought to nearby hospitals could not be admitted because of the damage to the hospitals. Windows and other installations of glass across the city were shattered.

Saint George Hospital University Medical Center before disaster

One of the city's largest medical facilities, was less than 1 kilometer from the explosion, and was so badly damaged that staff were forced to treat patients in the street. Four nurses died from the initial blast, fifteen patients died after their ventilators stopped working, and several child cancer patients were injured by flying glass. Within hours, after discharging all its patients and sending some to other facilities, Saint George Hospital was forced to close.

In the first week after the explosion, civilians gathered in hundreds to volunteer to clean up the debris on the streets and inside homes and businesses in Gemmayze, Achrafieh, and Karantina neighbourhoods. Many civil society organizations offered equipment and food to the volunteers, while many residents and businesses opened their homes and hotels for free to those who lost their homes in the blast.

On 6 August 2020, the Lebanese Forces Party's executive chairman Samir Geagea was the first politician to visit Beirut and launched from there a relief committee, Ground-0, under the leadership of the former minister Dr. May Chidiac to support in rebuilding Beirut. In December 2020, the committee achieved repairing 709 houses, assisted 5300 individuals and 2300 families, distributed 14000 food rations, made 2540 medical consultations, and provided 2030 individuals with medicine. In addition, the committee distributed more than 150 scholarships for Beirut schools' students.

This is the second time that Reguero has been shot. The first attack was in 2022 in zone 3 of the capital.

Los Angeles police released body camera footage Tuesday of a confrontation in September where a man accused of domestic violence fatally shoots and kills himself.

The incident occurred on Sept. 15 around 2: 50 p.m. outside the Los Angeles Police Department's Rampart Division station.

Police said officers confronted an armed domestic violence suspect, identified as Tyler Evans, a Supercross driver who went by the name "One Punch."

The day before Evan's death, police say Evans' girlfriend reported an incident of domestic violence to officers assigned to the Rampart station. Evans was not at the scene at the time the report was made.

The confrontation started when Evans drove his girlfriend to the Rampart station to discuss the domestic violence investigation. She told officers Evans was waiting for her outside the station with a gun.

Police said the man was wanted for domestic violence and refused to listen to their commands.

An officer yelled, "'Tyler, get on your knees! Tyler, don't run! Get on your knees!'"

Despite the commands, Evans appeared to back away, and officers used a bean bag gun. Moments later, police say Evans turned the gun on himself and took his own life.

Bruce Thomas, a retired officer, said the officers did everything right.

"They knew he was a dangerous person, so they tried to stop him using verbal commands," Thomas said. "You notice they have one officer doing that so there's no confusion."

Evans' sister said on social media that their father recently died, and that may have contributed to Evans' suicide.
