After a rabies exposure, the rabies virus has to travel to the brain before it can cause symptoms. This time between exposure and appearance of symptoms is the incubation period. It may last for weeks to months. The incubation period may vary based on

the location of the exposure site (how far away it is from the brain),

the type of rabies virus, and

any existing immunity.

The first symptoms of rabies may be similar to the flu, including weakness or discomfort, fever, or headache. There also may be discomfort, prickling, or an itching sensation at the site of the bite. These symptoms may last for days.

Symptoms then progress to cerebral dysfunction, anxiety, confusion, and agitation. As the disease progresses, the person may experience delirium, abnormal behavior, hallucinations, hydrophobia (fear of water), and insomnia. The acute period of disease typically ends after 2 to 10 days. Once clinical signs of rabies appear, the disease is nearly always fatal, and treatment is typically supportive. Less than 20 cases of human survival from clinical rabies have been documented. Only a few survivors had no history of pre- or postexposure prophylaxis.

The signs, symptoms, and outcome of rabies in animals can vary. Symptoms in animals are often similar to those in humans. These include early nonspecific symptoms, acute neurologic symptoms, and ultimately death.

Fibrodysplasia ossificans progressiva is a connective tissue disorder, in which fibrous connective tissue, like ligaments, tendon and muscle are replaced by bone, causing the patient to stiffen up. This is the only medical condition known to cause one organ to grow into another.

Fibrodysplasia ossificans progressiva, or FOP, is extremely rare and estimated to affect 0.5 cases per million people. In 2017 there were only 801 cases known worldwide.

The cause of this disorder is a mutation of the gene, ACVR1. It is the body's repair mechanism, making it use bone to repair instead of the normal tissue. This causes joins to fuse and become immovable if even minor trauma occurs.

The new bone formation is called "heterotopic ossification," that new skeleton eventually restricts the patients ability to move, keeping them in a fixed position for the rest of their life.

Even if the excess bone is surgically removed, the body will repair itself with what is refered to as an explosive growth of new bone. There is no cure for this condition.

Lymphoma of the eyelid and leg

We report the case of a 40-year-old patient, with no particular history, who has been experiencing swelling of the right upper eyelid for 3 months, gradually increasing in volume (A), associated with a lesion of the soft tissues of the posterior aspect of the leg. right (B). The ophthalmological examination revealed a large mass in the right upper eyelid at the expense of the lacrimal region, measuring 9 cm in long axis, necrotico-hemorrhagic reducing the eyelid fissure with visual impact. Best corrected visual acuity is 5/10 in the right eye and 10/10 in the left eye. Slit lamp examination finds cellular hyalitis 3 x. The fundus (FO) is unremarkable and the examination of the adelphic eye is normal. An orbito-cerebral and thoraco-abdominal scan was requested revealing a lesion process in the right lacrimal gland, poorly limited by contrast, with mediastinal lymphadenopathy of territory V. A tuberculin intradermal test (IDR) and an x-ray pulmonary came back negative. A biopsy of the lacrimal gland revealed a diffuse dermal infiltrate with round cells suggestive of a lymphomatous process and the immunohistochemical complement made it possible to establish the diagnosis of a primary cutaneous T lympho-proliferation with CD4 positive medium cells. Note that a biopsy of the lesion located in the leg was also carried out and the anatomo-pathological result returned identical to that of the lacrimal gland. The patient benefited from polychemotherapy with good post-therapeutic progress.

CEPHALIC PRESENTATION

CRANIOTOMY

Perforate the skull with perforator or a pair of heavy scissors

In face presentations use an eye as entry point

Open the shanks of the perforator

Break all intracranial septa

Apply the basiotribe with the solid leg inside and fenestrated leg outside of the skull

Be careful not to catch part of cervix or vagina in the grip

Tighten grip as much as possible

Extract the fetus.

Do this slowly

Allow time for the head to collapse.

In lack of a basiotribe use:

Ordinary delivery forceps

Several heavy toothed clamps

Remove the placenta manually

Check with a hand in the uterine cavity for rupture
