Kottayam, India. A 30 years old male patient was brought to Oral and Maxillofacial surgery casualty of Medical College Kottayam with a penetrating facial injury by a wooden log passing through the left lateral side of his face below the angle of the mandible and protruding through the oral cavity causing comminuted fracture of the mandible after a road traffic accident. There was moderate bleeding and respiratory distress. No history of vomiting or loss of consciousness. From the history, it was noted that the wooden branch was initially of the length of 8 feet. In the local hospital, the wooden log was shortened to 2 feet.

At the time of admission the patient complained of dyspnea and dysphasia. On examination there was significant oronasopharyngeal bleeding with no neurological deficit. A moderate trickling of blood from the entry site was noted. The wooden log was seen protruding through his mouth. The management of the patient was done by following the advanced trauma life support (ATLS) guidelines established by the American College of Surgeons, which begins with a rapid assessment of the airway, breathing, and circulation. The patient was unable to lie in a supine position due to the wooden log stuck behind the neck. Tracheotomy was performed under sedation and local anesthesia. General anesthesia was then administered. There were no other associated injuries.

A midline lip-split incision was given till the anterior extension of the penetrating wound. Exploration was performed through existing mandibular symphysis fracture. Comminuted fracture of the left body of the mandible and symphysis were identified. Access was gained by swinging the mandible through the fracture symphysis (Fig.4). After adequate exposure, the wooden log was removed in toto (as a whole) (Fig.5). There was no injury to great vessels and nerves of the neck. Reduction and fixation of fracture mandible were done by stainless steel mini pates and screws (Fig.6). The wound was then closed in layers after securing the suction drain. CT scan revealed that there were no associated head injury, cervical spine injury, maxillofacial injury, or any other foreign body.

The postoperative evaluation showed no injury to the facial and lingual nerve. The patient had an uneventful postoperative period and was discharged on the 10th day with the tracheostomy tube removed. He was followed up regularly. A 1-month review showed no functional or major aesthetic problems (Fig.7).

Penetrating injuries of the face and neck though uncommon are potentially life-threatening entities. Many cases of penetrating neck injuries that required massive surgical intervention and repair have been reported. Initial clinical presentations sometimes may not reflect the true injury that has occurred internally. A clinically superficial-looking minor stab wound can have a vascular or aerodigestive injury when the wound is explored.

This individual who was an alcoholic was found dead, and in the bloating stage of decomposition. The t-shirt reads "I got this way from drinking beer" and seems to be quite accurate, but I have no further info about this case or cause of death. Sorry for low image quality.

At the bloating stage, the microorganisms residing in the human gut begin to digest the tissues of the body, excreting gases that cause the torso and limbs to bloat, and producing foul-smelling chemicals including putrescine and cadaverine. The gases under the skin also causes the skin to turn green and black. Cells in tissues break down and release hydrolytic enzymes, and the top layer of skin may become loosened, leading to skin slippage. Decomposition of the gastrointestinal tract results in a dark, foul-smelling liquid called "purge fluid" that is forced out of the nose and mouth due to gas pressure in the intestine. The bloat stage is characterized by a shift in the bacterial population from aerobic to anaerobic bacterial species.

A 29-year-old student from Germany was found dead in a hotel room on the last day of his 3-day biking tour at a Slovak mountain resort. I might have posted this before, but I redid the images, and they are much sharper now -Herman. According to hotel staff, he had been unaccompanied, he was only in obligatory contact with the reception desk and was last seen in the evening several hours prior to his death. His undressed body was found by a cleaning staff lying on a bed in the supine position with his upper arms extended sideways (Fig. 2).

There was a full-face diving mask (CRESSI-SUB, Genoa, Italy) found on the face of the victim, being snugly sealed by rubber straps (Fig. 1). The glass of the full-face mask was covered with tiny droplets of the condensed vapor on the inner side. The full-face diving mask was connected with a second stage of standard open-circuit scuba (self-contained underwater breathing apparatus, NOAA) diving regulator.

The 2nd stage of the above-mentioned regulator was connected via a flexible medium-pressure rubber hose with the 1st stage of the scuba breathing system used. Finally, the 1st stage of the regulator was firmly screwed into the orifice of a valve of a high-pressure industrial gas tank, filled with pure nitrogen. Both regulators were in the open position.

The cause of death was attributed to nitrogen asphyxiation. Further investigation provided by the German police revealed that the deceased man had made suicidal declarations in the past and that the diving equipment was his own. Further- more, his close relatives confirmed suicidal ideations of the deceased; however, an apparent reason remained unclear. Following the completion of the police investigation (both German and Slovak), the manner of death was classified as a suicide.

The internal examination revealed the following findings: the brain of the deceased was swollen with dilatation and congestion of blood vessels in the leptomeninges. Both lungs were enlarged, congested, and edematous (left lung weight 750 g; right lung weight 950 g). Tiny hemorrhagic foci were found on the pleura of both lungs (Fig. 4). There was a pinkish mucous froth in the airways. The heart was excessively dilated, and the muscular walls were flaccid, with a total loss of tonus. The abdominal organs were congested, and the liver was slightly enlarged (1670 g) without, however, visible morphological changes. The blood within the whole body was dark red and fluid. The reminder of autopsy was unremarkable.

Ted White dies at 92

Best known to horror fans for playing Jason Voorhees in Friday the 13th: The Final Chapter, we’ve learned the sad news tonight that actor/stuntman Ted White has passed away.

Ted White was 96 years old.

Convention All Stars owner Sean Clark writes on social media tonight, “As I’m here setting up at Monsterpalooza I just got hit with the news that my dear friend Ted White has passed away. I was told he passed away peacefully in his sleep at his home.

“I’ve had the pleasure of knowing him and working with him for close to 20 years. He was my adopted grandpa and hands down the best storyteller you could have ever met. If you haven’t read it yet check out his book Cast a Giant Shadow where many of those stories are told. I was honored to be mentioned in the book when he wrote about his time in the convention world. I am going to miss you immensely and those steak dinners we always had. Also teasing you about your red, white & blue banner that made you look like you were running for President.

“My heart goes out to his wife Jeri and his sons. It was an absolute honor my friend. Rest easy. Love you Ted.”

Ted White doubled for everyone from Clark Gable to John Wayne and Lee Marvin, and many horror fans consider his Jason Voorhees to be the definitive depiction of the character.

In addition to Friday the 13th: The Final Chapter, Ted White also did stunt work for countless films including Creature from the Black Lagoon, Giant, Rio Bravo, Planet of the Apes, King Kong, The Manitou, Escape from New York, Road House, and Gone in 60 Seconds, and as an actor he appeared in TRON, Romancing the Stone, Starman, The Hidden, and “The X-Files.”

Robbie Coltrane Dies at 72

Actor Robbie Coltrane, who played Hagrid in the Harry Potter films, has died aged 72.

He also appeared in ITV detective drama Cracker and the James Bond films Goldeneye and The World Is Not Enough.

In a statement, his agent Belinda Wright confirmed the actor died in hospital near Falkirk in Scotland.

She described Coltrane as a "unique talent", adding his role as Hagrid "brought joy to children and adults alike all over the world".

"For me personally I shall remember him as an abidingly loyal client. As well as being a wonderful actor, he was forensically intelligent, brilliantly witty and after 40 years of being proud to be called his agent, I shall miss him."

"He is survived by his sister Annie Rae, his children Spencer and Alice and their mother Rhona Gemmell. They would like to thank the medical staff at Forth Valley Royal Hospital in Larbert for their care and diplomacy."

"Please respect Robbie's family's privacy at this distressing time."

Coltrane was made an OBE in the 2006 New Year's honours list for his services to drama and he was awarded the Bafta Scotland Award for outstanding contribution to film in 2011.

Harry Potter star Daniel Radcliffe paid tribute to Coltrane in a statement, saying: "Robbie was one of the funniest people I've met and used to keep us laughing constantly as kids on that set.

"I've especially fond memories of him keeping our spirits up on Prisoner of Azkaban, when we were all hiding from the torrential rain for hours in Hagrid's hut and he was telling stories and cracking jokes to keep morale up.

"I feel incredibly lucky that I got to meet and work with him and very sad that he's passed. He was an incredible actor and a lovely man."

Fellow Harry Potter star Emma Watson said: "Robbie, if I ever get to be so kind as you were to me on a film set I promise I'll do it in your name and memory."

She paid tribute to Coltrane on Instagram saying there was "no better Hagrid" and he "made it a joy to be Hermione".

"I'll really miss your sweetness, your nicknames, your warmth, your laughs and your hugs."

Writing on Twitter, Harry Potter author JK Rowling described Coltrane as an "incredible talent" and "a complete one-off".

Actor Stephen Fry, who appeared alongside Coltrane in Alfresco, tweeted: "Such depth, power and talent: funny enough to cause helpless hiccups and honking as we made our first TV show Alfresco. Farewell, old fellow, you'll be so dreadfully missed."

Fellow Alfesco actor Hugh Laurie, who also starred alongside Coltrane in Blackadder, recalled their time spent sharing car rides between Manchester and London. "I don't think I've ever laughed or learned so much in my life", he tweeted.

And Blackadder star Tony Robinson described Coltrane as "such a sweet man... so talented as a comic and as a straight actor". Posting on Twitter about his favourite episode from the period sitcom, he said: "It was all down to you mate."

Scotland's First Minister Nicola Sturgeon described Coltrane's death as "very sad news".

"He had such range and depth as an actor, from brilliant comedy to hard-edged drama. I think my favourite of all his roles was Fitz in Cracker," she said. "Robbie Coltrane, Scottish entertainment legend - you will be hugely missed. RIP."

The official James Bond Twitter account called him an "exceptional actor whose talent knew no bounds".

Broadcaster Richard Coles added: "Very sorry to hear Robbie Coltrane has died. We shared a dressing room once and he had the biggest pants I have ever seen, which he wore with tremendous flair. We were friends from then on."

The Scottish star, whose real name is Anthony Robert McMillan, was born in Rutherglen, South Lanarkshire, in 1950.
